Greene County restaurant inspections for Nov. 12-17, fish not labeled properly on menu

Food establishments in Greene County are inspected one to three times per year, depending on the type of food served, the population served, the difficulty level of food preparation, and past history.

Issues found during inspections fall into two categories: priority and non-priority. Priority violations are issues that can have a direct impact on the safety of the food and can lead to a restaurant getting shut down if they persist. Non-priority violations are usually centered around things like sanitation or building maintenance.

Here is information from the Springfield-Greene County Health Department regarding restaurant inspections over the past seven days:

Arby’s Roast Beef Restaurant #8274, 230 S. West Bypass. 11/16/2021 – Routine Inspection; Result: Active. Jennifer was person in charge at time of inspection. Return air is getting some buildup and should be cleaned. No violations cited Priority Violations Found: 0. Nonpriority Violations Found: 0.

Atrium Bakery, Cox South, 3801 S. National Ave. 11/12/2021 – Routine Inspection; Result: Permitted with Reinspection. Discussed requirement for ingredient labels on prepacked ready-to-eat foods. Priority Violations Found: 2. Observed: Foods held at temperatures above 41F in prep cooler. Observed: Several ready-to-eat potentially hazardous foods not marked with discard date. Nonpriority Violations Found: 0.

Bob & Ikes Neighborhood Bar, 1946 S. Glenstone Ave. 11/16/2021 – Routine Inspection; Result: Active. No violations cited at time of inspection. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Bubba’s BBQ, 504 N. West Bypass. 11/16/2021 – Compliance Check 1; Result: In Compliance. Priority Violations Found: 0. Nonpriority Violations Found: 2. Observed: Lights in hallway storage area and in walk-in cooler found not shielded. Observed: Seal on door of the hot hold cabinet missing in some places and in poor condition.

Captain D’s #3765, 2604 E. Sunshine St. 11/15/2021 – Routine Inspection; Result: Active. Priority Violations Found: 1. Observed: Potentially Hazardous food (Rice 44F) not being held at 41F or below. Item was voluntarily discarded. Corrected on site. Nonpriority Violations Found: 0.

Casey’s General Store #3010, 325 W. Cardinal Drive. 11/12/2021 – Routine Inspection; Result: Active. Discussed requirement to mark discard time on time only controlled foods. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Creasian, 501 W. Chestnut Expressway. 11/12/2021 – Suspended Permit Reinspection; Result: Approved. On site to reopen facility after plumbing backup had been resolved. Facility will have compliance check in the coming weeks to verify violations noted at last inspection are under control. Facility approved to reopen. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Domino’s Pizza, 3308 W. Chestnut Expressway. 11/16/2021 – Routine Inspection; Result: Active. No violations noted during the inspection. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Eugene Field School, 2120 E. Barataria St. 11/15/2021 – Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Evangel University Cafeteria, Crusader Hall. 11/16/2021 – Routine Inspection; Result: Active. No violations cited at time of inspection. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Evangel University Student Union, The Joust. 11/16/2021 – Routine Inspection; Result: Active. Priority Violations Found: 1. Observed: Potentially Hazardous Foods not being stored at or below 41F(Sour cream 47F, Lettuce 48F). Items were voluntarily discarded. Corrected on site – The unit is being placed on time control. Nonpriority Violations Found: 0.

Flat Creek Bar And Grill, 772 E. Highway 60, Republic. 11/15/2021 – Routine Inspection; Result: Active. Discussed: Consumer Advisory must be on menu with a marking next to any items offered undercooked linking it to the consumer advisory (i.e. burgers, steak, eggs); be sure all sprayers are labelled, including food items in sprayers; all surfaces must be smooth and easily cleanable. Priority Violations Found: 2. Observed brisket at 43F(wrapped in plastic wrap) in walk-in cooler. Employee said food had been cooked yesterday and immediately wrapped in plastic, then placed in walk-in cooler to cool. Discussed not immediately wrapping items, but allowing them to cool to 41F before wrapping. Facility may choose to portion into smaller portions to speed up the cooling process. Brisket discarded voluntarily by person in charge. Observed in portioned ranch at 45F, and portioned sour cream at 45F in server prep cooler. Date marking indicated it had been prepped yesterday. No temperature logs to verify items had been in temperature today. Person in charge voluntarily discarded sour cream and ranch. Discussed keeping portioned items in bottom of prep table or using metal lid to cover in top of cooler. Items must be held at 41F or below. Nonpriority Violations Found: 1. Observed swai being advertised and sold as catfish on menu. Food must be honestly presented. Catfish may be obtained and sold as catfish or menus can be changed to signify fried fish instead of catfish.

Gailey’s Breakfast Cafe, 220 E. Walnut St. 11/17/2021 – Routine Inspection; Result: Active. Katelyn was person in charge at time of inspection. Priority Violations Found: 1. Observed: Quaternary ammonia solution at bar sink was less than 100 ppm quaternary ammonia. Corrected by draining and refilling sink with correct solution. Nonpriority Violations Found: 1. Observed: Some pans were stacked prior to being dry. This can increase chance of bacteria growing in the wet environment.

George’s Steak House, 339 S. Glenstone Ave. 11/17/2021 – Routine Inspection; Result: Active. Priority Violations Found: 1. Observed: Dented can of olives found stored on the shelf. Education given and corrected during inspection. Nonpriority Violations Found: 1. Observed: Missing or worn ceiling tiles in kitchen.

Godfather’s Pizza, 5453 S. State Highway FF, Battlefield. 11/16/2021 – Routine Inspection; Result: Active. Discussed: 1. Do not double bin in top of prep cooler 2. Service animal questions that may be asked are: Is this a service animal? What service does the animal provide? Priority Violations Found: 0. Nonpriority Violations Found: 1. Observed employee wash hands in ware washing sink. Corrected by having employee discard pizza dough touched afterward. Employee washed hands in hand wash sink before returning to work.

Hickory Hills School, 4650 E. Division St. 11/12/2021 – Routine Inspection; Result: Permitted with Reinspection. Priority Violations Found: 1. Observed double door reach-in cooler on elementary side holding cheese sticks at 53F. Cheese sticks voluntarily discarded. Pizza prepped today moved to alternate cooler. Manager placed a work order in for maintenance immediately and agrees not to store any PHFs in unit. Nonpriority Violations Found: 0.

Hickory Hills School, 4650 E. Division St. 11/15/2021 – Re-Inspection; Result: Active. Priority item corrected. Reach-in cooler now holding below 41F as required. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Holiday Inn & Suites Springfield, 2720 N. Glenstone Ave. 11/15/2021 – Routine Inspection; Result: Active. Priority Violations Found: 1. Observed open package of sliced ham in walk-in cooler without a date mark date of when the ham was opened. Corrected at time of inspection. Nonpriority Violations Found: 0.

Houlihan’s South, 2110 E. Republic Road. 11/12/2021 – Re-Inspection; Result: Active. At the time of the reinspection, the make table unit was properly cold holding temperatures. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Kickapoo High School, 3710 S. Jefferson Ave. 11/15/2021 – Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Lucy’s Chinese Food, 2456 E. Sunshine St. 11/15/2021 – Routine Inspection; Result: Active. Priority Violations Found: 1. Observed: Potentially hazardous foods not being cooled properly to 70F within 2 hours. (Rice 98F, Noodles 47F) Items were voluntarily discarded. Corrected on site. Education was given. Mexican Villa Comida, 434 S. Glenstone Ave. 11/16/2021 – Re-Inspection; Result: Active. Violations corrected, baseboard and wall panelling is sealed and no live pests observed at time of inspection. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Pleasant View School, 2210 E. State Highway AA. 11/15/2021 – Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Rib Crib #19, 1640 N. Glenstone Ave. 11/12/2021 – Re-Inspection; Result: Permitted with Reinspection. Core item extension given. Water leak is minimal, however it is not raining today. No standing water observed in walk-in cooler at time of visit. Priority Violations Found: 0. Nonpriority Violations Found: 1. Observed water dripping into walk-in cooler at door. Some material has been replaced on walk-in cooler, however the roof is still leaking and water is coming through from the top of the door frame.

Ruby Tuesday #7888, 2725 N. Glenstone Ave. 11/12/2021 – Re-Inspection; Result: Active. Priority violations were corrected prior to the re-inspection. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Shady Dell School, 2757 E. Division St. 11/15/2021 – Routine Inspection; Result: Active. Priority Violations Found: 0. Nonpriority Violations Found: 0.

Triple Eight Chinese Restaurant, 1710 S. Kansas Expressway. 11/17/2021 – Routine Inspection; Result: Active. Priority Violations Found: 1. Observed: Cooked chicken in walk-in cooler with no date labeling. Education given and corrected during inspection. Nonpriority Violations Found: 3. Observed: Miscellaneous items blocking employee access to hand washing sink. Observed: Vent in walk-in cooler found not clean. Observed: Can good items, bags of flour and bags of rice in shipping packaging still found stored directly on the floor.

Watkins School, 732 W. Talmage St. 11/16/2021 – Routine Inspection; Result: Active. Priority Violations Found: 1. Observed: Two dented cans of sliced peaches on the shelf with other canned goods. Education given and items removed. Nonpriority Violations Found: 0.
